Responsible for creating, completing, auditing, and accurate scanning of patient medical records. Responsible for ICD-10 and CPT coding.

Qualifications

High school diploma or equivalent required. CPC, CCS or CMC Required. Coding experience preferred. Two (2) years’ experience in medical records, health information, or document management preferred. Must be computer literate, detail-oriented, have multi-tasking abilities, and maintain accuracy under scrutiny and stringent timelines. Must be a team player, work well with others, be willing to learn new things and accept change, demonstrate good interpersonal communication skills, and be self-motivated.

Key Result Areas

• Responsible for timely and accurate data entry for the agency.

• Experience using MS Word, Excel, Outlook.

• Experience working with an EMR system

• Verification of incoming and scanned documents to ensure compliance.

• Completion of time sensitive patient record information.

• Timely chart preparation for retrieval and scanning.

• Assist HIM internal and external customers with requests of medical documentation.

• Keep track of all documentation leaving the department.

• Participate as an active supporter of the Treasure Coast Hospice compliance program, including an emphasis to report, as well as complying with all Federal, State and Local laws as well as the organization’s compliance program.

• Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and assigned.

• Maintain a high level of urgency, customer consciousness and service excellence.

• Responsible for the data entry of diagnosis (ICD-10) codes as well as auditing Providers visits to ensure the correct level of service codes are on the claim.

This position will be categorized as category III (involves no routine or potential exposure to blood, bodily fluids/tissues).

Working Conditions

• Picking, pinching, typing or otherwise working, primarily with fingers rather than with the whole hand as in handling.

• Talking. Expressing or exchanging ideas by means of the spoken word. Those activities in

which they must convey detailed or important spoken instructions to other workers

accurately, loudly, or quickly.

• Hearing. Perceiving the nature of sounds at normal speaking levels with or without

correction. Ability to receive detailed information through oral communication, and to

make the discriminations in sound.

• Light work. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of

force frequently, and/or a negligible amount of force constantly to move objects. If the use

of arm and/or leg controls requires exertion of forces greater than that for sedentary work

and the worker sits most of the time, the job is rated for light work.

• The worker is required to have close visual acuity to perform an activity such as: preparing

and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading;

visual inspection involving small defects, small parts, and/or operation of machines

(including inspection); using measurement devices; and/or assembly or fabrication parts of

distances close to the eyes.

• The worker is not substantially exposed to adverse environmental conditions (such as in

typical office or administrative work).

Additional Information

Incumbent may be required to perform emergency duty before, during and/or beyond normal work

hours or days in the event of an emergency, crisis situation or disaster (man-made or natural)

including evacuation sites. The person in the position will work in a smoke-free location, and is

expected to adhere to all smoking restrictions.
