Cascade Community Healthcare (CCH) has been providing treatment services to Lewis County and surrounding areas for more than 50 years. Our mission is to provide our clients with compassionate and encouraging counseling leading to a renewed sense of hope and the ability to make positive changes and live more satisfying lives. Cascade currently has six treatment locations in Lewis County.


We are looking for a Psychosocial Rehabilitation Counselor to join our Assertive Community Treatment (ACT) Team. The Psychosocial Rehabilitation Counselor has the responsibility to carry out rehabilitation and support functions and assist in treatment with specific focus on identifying and supporting issues of recovery and ADLs.  In addition, they will provide peer services to individuals with severe and persistent mental health disorders who are most at risk of hospitalization, incarceration, and/or homelessness due to their mental health symptoms. 


SHIFTS AVAILABLE:

This position will normally require that the employee work a non-standard rotation schedule including weeknights and weekends, including on-call rotations.  This position is classified as being “non-exempt” according to the Fair Labor Standards Act.


PAY SCALE:

$23.50 - $28.65 Hourly. DOE.


WHAT YOU NEED TO GET THE JOB DONE:

  • Bachelor’s degree in psychology, social work, or closely related field required.
  • Two years’ experience working with individuals with sever and persistent mental health disorders preferred.
  • Previous experience working as a member of a multidisciplinary team and working collaboratively with other service providers preferred.
  • WA State Department of Health Agency Affiliated Counselor (AAC) credential required.
  • Experience in medical, mental health or social services field preferred.
  • Must be able to pass WA State Patrol background checks, national background check may be required.
  • Must have valid driver’s license and must be able to legally operate a motor vehicle.
  • Basic knowledge of money management and good math skills.
  • Ability to maintain a positive, solution-focused demeanor when responding to conflicts or problems.
  • Demonstrated organizational skills and the ability to multitask within a fast-paced environment.
  • Excellent communication skills, written and verbal.
  • General computer proficiency, including the ability to utilize electronic medical record systems and accurate typing and word processing skills.
  • Must be able to represent the agency in a positive and professional manner.
  • Must maintain current home telephone and address on file with the Human Resources office.


WHAT YOU'LL DO:

  • Provide services aimed at reducing symptoms/behaviors associated with a mental health diagnosis. Helping individuals to be successful and satisfied in the living, learning, working and social environments of their choice.
  • Assist clients in educational, work skills, sobriety or with other issues related to their diagnosis by obtaining skills to live independently or by preventing decompensation or a more restrictive situation.
  • Document identifying client symptoms or behaviors and interventions aimed at assisting client with needs.
  • Assist clients in gaining and utilizing skills necessary to participate in community reintegration- educational groups, social skills, communication skills, ADLs support, successful interpersonal behavior, and coping skills.
  • Observe clients, provide support, positive reinforcement, teach and model appropriate social skills.
  • Assist clients in the following Psychosocial Rehabilitation Domains:
    • Living: Includes understanding and communicating, maintaining, or sustaining housing, and performing essential tasks of community living (including shopping, cooking, cleaning, budgeting, etc.).
    • Learning: explore education goals, for example obtaining GED or enroll in training course.
    • Working: any goal of working, earning money, or obtaining sills to do so. Coordinate with vocational education specialist within team.
    • Socializing: enhancing social life; acquiring or improving leisure and recreational activities. Improving relationships with natural supports.
  • Involvement in the prevention of escalation, crisis intervention, decompensation and all necessary documentation following such events.
  • Report changes in client physical conditions/behaviors.
  • Medication monitoring as directed. 
  • Must meet productivity/direct service requirements as assigned by program manager.
  • Participates in various committees and meetings related to department activities and responsibilities.
  • Attends departmental and facility committee meetings as assigned.
  • Enter and extract client data as required from Electronic Health Record.
