SUMMARY: Peer Navigators have lived experience in substance use. Peers work with individuals who use substances and are seeking assistance in recovery services. Services can range from medication assisted treatment, in patient treatment, to sober living opportunities. All services focus on harm reduction and trauma informed care.

DUTIES AND RESPONSIBILITIES:

  • Identify and address barriers to necessary services
  • Provide emotional support to clients
  • Coordinate care among providers to address client needs
  • Complete required paperwork for each client file
  • Document all services in the AWARDS data base
  • Provide health literacy education to clients in group and individual sessions
  • Assist clients with medication financing and management
  • Coordinate care services among providers, addressing client needs
  • Complete referrals in a timely manner
  • Effectively communicate any incidents and/or crisis information to supervisors immediately
  • Maintain records and case files, including individuals’ personal information, services provided, and relevant correspondence
  • Resolve client conflicts, escalating them to management when needed
  • Meet with program manager daily ;submit a daily shift report which includes all tasks performed as well as any issues that arose during the shift
  • Develop and maintain relationships with clients and encourage them to maintain a positive attitude
  • Analyze information pertaining to individual service plans and clients progress
  • Assist in data entry for computerized client tracking
  • Attend all required staff meetings and trainings
  • Other duties as assigned by supervisor
  • Performs all other duties as assigned and required.

QUALIFICATIONS:

Education/Experience

  • High school diploma or general education degree (GED); or one to three months related experience and/or training
  • Computer skills required: (Word); Electronic Mail Software (Outlook)
  • Other skills required:
    • Ability to remain calm in crisis, hostile, or disaster situations.
    • Ability work effectively and independently.
    • Strong coordination, typing and oral communication skills.
    • Bilingual Spanish Preferred
