Pay Rate: $20.00/hr - 22.00/hour

Position Purpose: Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services .
Job Duties:
- Develop, assess and adjust, as necessary, the care plan and promote desired outcome
- Assess member's current health status, review past and present treatment plan and services
- Coordinate services between Primary Care Physician (PCP), specialists, medical providers, etc.
- Develop plan of care based upon assessment with specific objectives, goals and interventions
- Facilitate member access to community based services
- Monitor referrals made to community based organizations, medical care and other services
- Enter and maintain assessments, authorizations, and pertinent clinical information into various medical management systems
Education/Experience:
- Bachelor’s Degree, Associates Degree or 3 years or more of health related experience.
- 2+ years of clinical nursing experience in a clinical, acute care, or community setting. Knowledge of healthcare and managed care preferred.
- Must be Bilingual fluent in both English and Spanish
- Knowledge of healthcare and managed care preferred.
- Prior case management experience preferred.
Certifications/Licenses:
- Current and valid CPR certification.
- Must be fully vaccinated and or willing to be vaccinated.
