Requirements for a Social Worker:
- Bachelor's degree in Social Work (BSW) or Human Service field
- CSW - Certified Social Worker license in state of New Jersey
- Proficiency using a clinical software system
- Understanding of MDS, care planning and admission/discharge processes
- 1 year or more of healthcare/long term care experience
Responsibilities for a Social Worker:
- Oversees residents in the skilled nursing setting by identifying their psychosocial, mental and emotional needs.
- Provide, develop and/or aide in access of services to meet resident needs.
- Foster a climate that provides both long term care residents and short term rehab residents with the highest practical level of physical, mental and psychosocial well being and quality of life.
