Tact Staff is seeking a Social Work Social Worker for a travel job in Arlington, Virginia.

Setting: Acute rehab unit. Must be strong with dc planning for rehab pts. This is strokes, amputations etc.

Duration: 13-weeks
Schedule: Mon-Fri with 2 weekend shifts per month
REQUIREMENTS:
• EPIC experience
• General Skills: High Risk Intervention Patient, Knowledge of Cultural Issues, NASW Code of Ethics/ Standards of Home Health Care, Patients with Communication Problems (Hearing/Speech), Community Programs (Income/Insurance), Community Based Service Programs, Adult Protective Services Child Protective Services, Emergency Detention Vulnerability or Safety Risk
• Supported Unit Experience: Medical Surgical Unit
• Assessment Experience: Assess Response to Treatment, Biopsychosocial Assessment, Conduct Initial Evaluation, Develop Plan of Care, Development of Problem List
• Care Coordination Experience: Coordinate Services - Home Care, Documents Information to Facilitate Care Coordination, Participates in Care Conference Activities, Refers to Rehab and Long-Term Care Facilities, Refers to Other Disciplines/Resources, Supervises Staff Effectively and Efficient
• Documentation/Communication Experience: Discharge Summary, Documentation According to Plan of Care, Documentation of Patient/Caregiver Teaching/Learning, Documentation Reflects Understanding of Coverage Criteria, Documents Patient Response
• Counseling/Therapy & Education: Facilitate Discharge/Contingency Planning
• Experience: 2 years preferred

Ratio: 18-20:1
EMR: EPIC (experience required)
Duties: responsible for the collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet an individual's health needs through communication and available resources to promote quality cost-effective outcomes
*EPIC: ad-hoc class
*No scrubs. Attire is casual wear, no opened toe shoes.
