Shift: Days / 8a-4:30p, 8x5, 40hrs per weekLength: 13 weeks
no weekends; no on call; scrubs (any color) and closed-toed shoes required
Requirements: 
  • 1 year Home Health experience
  • 3 months OASIS documentation experience
  • BLS
  • Psych Home Health; Tracheostomy care skills preferred
  • MatrixCare documentation preferred

Registered nurse; 1 year Home Health experience REQUIRED; >3 mo OASIS documentation experience 
Days 5x8s; 25 units productivity per week; no weekends; no on call; scrubs (any color) and closed-toed shoes required.

Certifications REQUIRED: BLS (AHA), HI License

Skills REQUIRED: Adult Home Health; Admission Assessment; Case Management; Discharge/discharge due to safety or Non compliance; Recertification; Resumption of care; Routine visit; Significant change in condition; Start of Care; Transfer; Blood Glucose/glucometer management; Colostomy/ileostomy management/care; G-tube site care; Management of feeding tubes; NG tube management/care; Nutritional Assessment; Post AV Fistula/Shunt; Suprapubic Catheter insertion/management; Urinary Catheter insertion/ management; Drain Management; Pressure ulcer prevention/management; Pressure ulcer staging; Wound care; Wound V.A.C management; Medication Assessment/Management; Pain Management/Assessment; Utilization of pain scales (verbal, nonverbal); Monitoring Chemotherapy side effects; Pain management/hospice; Port insertion/care; Cast/brace/immobilizer device care
Post amputation; Post Total Joint Replacement; Administration of blood/blood products; Care and management of peripheral IVs/ central lines/ports; IV start skills; Phlebotomy skills; Clinical assessment documentation; Coordinate services; Infection Control Precautions; Obtain Vital Signs; Oxygen safety/administration

Skills Preferred: Psych Home Health; Tracheostomy care; MatrixCare documentation
