Job DescriptionDoctor's Community is seeking a Full-Time Licensed Social Worker Enrich lives as a nursing center Social Services Specialist where you will work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and then obtain services to meet those needs. The goal is to provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life. Services are provided in accordance with the National Association of Social Workers (NASW) Code of Ethics and compliance with federal, state, and local guidelines and regulations, company policies and procedures, and standards of care. Position Highlights*Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
