Description

Eddy Visiting Nurse & Rehab Association has a rewarding opportunity for a full-time LPN to join our home care team. This is a dynamic position working directly with RN’s in the homecare setting. Be on the front-line of the transitions in health care from inpatient settings to the community – home health care is the future

• Excellent orientation provided with support from supervisory team.
• Care for ONE PATIENT AT A TIME.
• Experience the rewards of being able to address the whole patient,
• Help patients become independent with their treatments and self-management of their chronic illness(es),
• Once trained, enjoy the independence of starting and ending your day from home,
  
  

Work schedule ideally would be Monday to Friday, typically 8am to 4pm. Hours and days can be flexible. Minimal weekend and holiday rotations.

Requirements

• Valid NYS LPN License
• Must have 2 years LPN experience in acute care, rehab, or homecare
• Must be organized, able to teach, and be willing to travel from patient to patient.
• Must have strong communication with RN’s, other interdisciplinary team members and family members to be successful.
• Strong critical thinking skills.

Pay Range: $24.65 - $32.00

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
