Location: Boston, MA through Fall River, MA
Travel: patient service area is as far north as Boston, MA and as far south as Fall River, MA with mileage reimbursement (company cars to be available soon)
Schedule: Full-time, flexible Monday-Friday 8am-5pm

The Registered Nurse – Care Manager provides guidance and support to primary family caregivers on the Structured Family Caregiver program (SFC). The RN works to provide an assessment of the needs of the SFC caregiver and create a coaching plan. They also monitor and provide ongoing assessments, training, and support to the primary (family) caregiver and the client, adjusting as necessary. Duties to include:
-Provides assessment to ensure the Family Caregivers'
-Provides ongoing coaching and training to the Primary Family Caregiver to ensure all client goals are met
-Schedules periodic case conferences with the client, the Primary Family Caregiver, and the Medicaid Waiver Case Manager
-Sends summary reports to the Medicaid Waiver Case Manager portal as needed

REQUIREMENTS:
-Must have an active Registered Nurse license in Massachusetts
-At least 1 year of home health nursing experience or 3 years of geriatric nursing experience; experience with disabilities is a plus
-Must be comfortable with heavy travel time
-Knowledge of DDS / MassHealth Medicaid programs and home and community-based services (HCBS) is a plus
