Job Title: Wound, Ostomy, and Continence Nurse


Job Summary:

Under the direction of the Wound and Ostomy Manager, the Wound, Ostomy, and Continence Nurse will provide safe, competent, quality care based on nursing theory and research to a designated group of patients.


Duties and Responsibilities:


  • Conduct health assessments, develop care plans, and recommend treatments focused on wound, ostomy, and continence care.
  • Collaborate with the healthcare team, making referrals as needed, and participate in quality improvement activities.
  • Educate patients, families, and staff on preventing complications related to ostomies, wounds, or continence issues.
  • Assess and document patient responses to treatments and evaluate the quality of care provided by clinical staff.
  • 70% of the role will be in office, nurse might be required to travel to patients' home (MA, BKLYN, BX &QU) to complete wound care consults.


Skills and Experience:

  • Minimum two (2) years of experience in a Certified Home Health Agency or post-acute care facility.
  • Minimum of 2 years of Public/Community Health and 2 years medical/surgical experience required. Previous experience in PI, i.e., knowledge of PI applications, tools and techniques and knowledge of the development, coordination and facilitation of PI programs.
  • Previous experience using various computer software applications: i.e., Word, Excel, Access, etc.


Certifications and Licensure:

  • Current New York State Registered Nurse License
  • Certification as a Wound Care Nurse or Wound Ostomy Care Nurse

Pay Rate: $75.00 - $80.00 Per Hour

Shift and Time: 9A - 5P
