Job Description

Experience:

• 24 months of Hospice experience is required.

• Experience with Homecare Homebase (HCHB) is required.

Requirements:

• Candidates must have a Colorado license or compact license in hand at the time of application.

• This role will require floating to multiple locations

• Travel and Local candidates are eligible to apply for this role.

• COVID vaccination required for onboarding. Religious and medical declinations accepted.

Pre-employment modules may be required for this role. Please upload any certifications or health documents you have to your profile to expedite your on-boarding process.

Additional Details:

Schedule: Monday-Friday

No on-call requirement.

Will be serving the Loveland, Greeley, Fort Collins areas.

HomeCare HomeBase or Pointcare experience required. Will only consider without HCHB exp if CLN completes online training class prior to start.

Any overtime must be pre-approved.
