  • Facility: BayCare HomeCare 
  • Location: Largo, FL 
  • Status: Full Time 
  • Shift Hours: 8:00 AM - 5:00 PM, Monday - Friday
  • Weekend Work: Occasional 
  • On Call: Yes 
  • Home visits in assigned service area: Largo, St. Petersburg, Pinellas Park surrounding areas assigned by zip code.  

The Home Health Field, RN - hands on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include: 

  • Oversee the activities of the multi-disciplinary team that provides care to the patient 

  • Monitor the provision of therapeutic services through written and verbal communication with all disciplines 

  • Ensure that all visits scheduled and provided are authorized and covered by physician orders 

  • Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits 

  • Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor 

 Home Health RN Requirements: 

  • BLS (Basic Life Support) 

  • RN (Registered Nurse) License 

  • Driver's License State of Florida (Florida driver's license is required within 30 days of hire) 

  • Required Associate's or Diploma Nursing 

  • Required 1 year Registered Nurse or 1 year Licensed Practical Nurse 

  • Preferred Bachelor's Nursing 

  • Preferred 1 year Home Care
