Job Description

RN Home Health                                                   
 Responsibilities:



  • Open cases with assessment and develop a plan of care
  • Provide appropriate treatment to achieve positive patient outcomes
  • Supervise LPNs and HHAs
  • Educate patients and families to improve outcomes and ongoing care
  • Work closely with other team members and administrative staff
  • Complete all documentation timely and accurately

 Requirements:



  • A Registered Professional Nurse with current License
  • 1+ year nursing experience
  • Home Care nursing experience a plus
  • OASIS experience a plus
  • Med-surge experience a plus
  • Wound care and PleurX catheter experience desired
  • Ability to travel between patients' homes
