Job Description

The Emergency Medicine (EM) Pharmacist will provide support to the EM providers and nurses and offer patient-centered services in the emergency department.

Additional responsibilities include: 

  • Provide comprehensive pharmaceutical care to our patients including: 
    • clinical intervention
    • teaching and pharmacy representation on various committees
    • participation in daily patient care, including acute care resuscitations in adult and pediatric emergency department, attending code stroke, STEMI, and trauma activations, geriatric patient care rounds
    • medication order clarifications with the prescriber
    • document any changes in patient and pharmacy records and inform others of medication order changes
    • facilitate transitions of care for patients upon admission to ED or inpatient units and/or discharge 
    • monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions
    • provide pharmacokinetic consultations as required
    • prevent, detect, monitor, document, and report adverse drug reactions and medications errors
    • participate in the drug therapy management of medical emergencies, including the preparation of acute critical medications
    • promote the use of the formulary by converting non-formulary orders to formulary when possible
  • Oversee medication reconciliation technician program to promote safety and accuracy during transitions of care upon admission. 
  • Conduct patient education and counseling on medications.
  • Augment the education of pharmacists, prescribers and nurses on procedures and treatments related to cardiac surgery patients; serve as a preceptor and mentor for pharmacists, pharmacy residents and students
  • Participate in multi-disciplinary committees to support the appropriate use of medication therapy; conduct medication use evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization.

Must be a graduate of an accredited school of Pharmacy with Pharm D degree required. Completion of a PGY 1 and PGY 2 residency in emergency medicine pharmacy preferred. Board certification in pharmacotherapy (BCPS) and/or in emergency medicine (BCEMP) preferred. Current NJ Board of Pharmacy license required. Minimum of 1-year experience in a hospital practice setting.
