Job Description

General tasks and responsibilities will include:

  • Provides comprehensive medical and nursing care, including preventive and supportive health care services to patients primarily enrolled in the NYC H+H Community Health Worker Corps program.
  • Serves as a point of clinical contact and information for Community Health Workers; works closely with the Primary Care Physicians for patients enrolled in the Community Health Worker Program to ensure that clinical care for these patients is met.
  • Manages health problems and coordinates health care for patients. Identifies and diagnoses illnesses and physical conditions, and implements therapeutic and corrective measures.
  • Consistent with the established nursing practice, differentiates between normal and abnormal findings in the screening process to detect changes in health status that require physician’s intervention.
  • Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary.
  • Evaluates and interprets patient data, in order to plan and implement appropriate nursing interventions.
  • Consults with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment that will best support improved outcomes and patient satisfaction.
  • Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment and other pertinent information. Identifies health and medical risks and needs.
  • Develops, implements, and evaluates patient plan of care. Determines effectiveness and modifies as per clinical standards and practice protocols, where needed, to achieve medical and health goals.
  • Orders, conducts, and interprets diagnostic and laboratory tests. Identifies and may prescribe appropriate pharmacological and non-pharmacological interventions and therapies.
  • Participates in meetings and assists with other clinical and operational tasks related to the H+H Community Health Worker Corps Program.

Minimum Qualifications

For Assignment to Nurse Practitioner

  • A valid New York State license and current registration to practice as a Registered Professional Nurse; and
  • Current certificate as a Nurse Practitioner issued by the New York State Department of Education

(NYSED); and

  • A current certification issued by the State Education Department to prescribe medication.

For Assignment to Nurse Practitioner (Primary Care/Behavioral Health)

  • In addition to meeting the qualification requirements of Nurse Practitioner, must be assigned to a

Primary Care or Psychiatric/Behavioral Health setting

Departmental Preference

Strongly prefer National Board Certification in specialty in which you are assigned.
