Responsibilities
Under the direction of the director, the social worker provides professional social service and discharge planning services to patients and families. This position works closely with the case manager, LCSW, nursing staff and medical staff.

- Special Skills/Qualifications/Additional Training/Experience Required
- Must be able to read, write, speak and understand English.
