Communities of Care offers World Class Home Care with small community values. We're looking for a Full Time OR Part Time Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team. We have several clients available around the state.

- 21-Year-Old Male
- Call for more information on cares.
- Days Shifts Mon, Tues (7a - 5p), and Thurs (7a - 7p), weekends when needed.
Responsibilities:
- Administer nursing care to client
- Help maintain patients treatment plans
- Monitor and report changes in patient symptoms or behavior
- Maintain accurate patient medical records
- Provide advice and support to patients and their family members
Qualifications:
- Work from a place of caring and honoring the dignity of all clients.
- RN or LPN Minnesota License
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
