Open Door is seeking a full-time Patient Navigator to join their Care Coordination Program. The Patient Navigator participates in a patient-focused team approach to health care service delivery and provide support to other care coordination staff through activities such as data entry, documentation, outreach and coordination of care. This position is based in Ossining, NY.

Responsibilities
* Maintain electronic medical records in compliance with Open Door’s standards with high efficiency.
* Assist patients with scheduling and coordinating transportation.
* Outreach patients identified as due/overdue for chronic disease follow up care and those needing follow-up after an emergency room visit or in-patient stay.
* Enter and collect data for internal and external systems.
* Monitor and address high priority referrals.
* Assess patients social determinants of health and assists with connecting individuals to the appropriate resources.
* Review eligibility for Medicare patients identified as potentially eligible for Annual Wellness Visits.
* Request consult notes/reports from specialists offices and imaging centers.

*Qualifications*
* *Bilingual (English/Spanish) required.*
* *High School Diploma required *
* *Associates Degree or BS/BA preferred *
* Proficient in MS Office applications, including Excel and other database systems.
* Experience with community organizations and health education preferred.
* Patient-centric with empathy, excellent active listening and communication skills, engaging and efficient.
* Focused on collaborative team-based care.
* Responsive and comfortable working in a fast-paced, high accountability environment.
* Highly organized, and self-directed, able to multi-task.
* Effective in problem solving.
* Flexible.
* Excellent judgment and ability to prioritize resources where they will be most useful.
* Engaged in learning and continuous quality improvement.
* Outstanding written and verbal communications skills with a culturally diverse patient and staff population.
* Passionate about providing high quality care to low-income, under-served residents and those lacking access to health care.
* Possess experience in verbal and written communications and superior customer service skills.
* Must have valid NYS driving license and occasional access to a car.
