Thome PACE® serves Jackson, Lenawee, and Hillsdale counties providing participants a wide range of medical and supportive services in our Day Clinic located in Jackson, Michigan or in the comfort of their own homes. The Thome PACE team of healthcare professionals specialize in working with the senior community to help participants and their family members make positive healthcare decisions and receive the care they deserve.
What you'll do:
The Registered Dietitian applies advanced knowledge and skills independently and is a crucial member of the Interdisciplinary Team (IDT). The Registered Dietitian is supervised by the Lead Registered Dietitian.
Responsibilities include:
Evaluating participants
Nutritional status
Devising and executing nutritional care plans, which may include home delivered meals and nutritional supplements.
Assessing nutrient needs
Prescribing nutrition
Evaluating the effectiveness of care, and communicating nutritional information to healthcare professionals.
Works collaboratively with other IDT members.
Coordination of 24-hour care delivery
Participates as a member of the Interdisciplinary Team.
Attends meetings and takes part in participant care planning
As we age, we often require a helping hand and medical assistance. Thome PACE (Program of All-Inclusive Care for the Elderly) understands this need and provides a welcomed alternative to nursing home care for participants. The PACE model of care allows seniors aged 55+ to remain independent in their homes and communities while receiving the integrated social, wellness, and medical care services aging adults require.

What it Takes:
Bachelor’s degree or master’s Degree from an ACEND accredited program and completed the Dietetic Internship.
Passed the CDR exam and maintain Registration status in good standing. Those who are eligible to sit for the CDR exam may be considered.
Must be CPR certified or willing to obtain CPR within 90 days if offered employment.
Master's Degree plus approved work experience preferred.
Certified Diabetes Educator (CDE) status preferred.
Minimum of one (1) year working with the frail and elderly.
