Speech-Language Pathology CFY (Clinical Fellowship Year)

We are currently seeking a Speech-Language Pathology CFY to join our dedicated team. This position offers the opportunity to make a meaningful impact on the lives of pediatric and young adult patients by providing speech and language treatment and diagnostic services in the comfort of their homes.

Job Description: As a Speech-Language Pathology CFY, you will be responsible for developing skilled and high-quality care plans that address the unique needs of our patients and their families. You will perform evaluations, interpret assessment results, and collaborate with physicians to create and implement speech therapy treatment plans. Additionally, you will assist patients in developing or regaining cognitive, speech, language, feeding/swallowing, and social/emotional functioning to improve their level of independence.

Responsibilities:

Perform comprehensive evaluations to assess speech, language, cognitive, feeding/swallowing, and social/emotional functioning.
Develop individualized treatment plans based on assessment results and patient/family goals.
Provide speech and language therapy services to pediatric and young adult patients in their homes.
Monitor and document patient progress, adjusting treatment plans as necessary.
Collaborate with physicians, caregivers, and other healthcare professionals to ensure coordinated and holistic care.
Educate patients and families on speech therapy techniques and strategies for improving communication and swallowing skills.
Adhere to all regulatory requirements and documentation standards of home health agencies.
Maintain confidentiality of patient information and uphold ethical standards of practice.

Qualifications:

Master's degree in Speech-Language Pathology from an accredited program.
Completion of clinical fellowship year (CFY) and eligibility for state licensure.
Ability to work independently and manage time effectively in a home health setting.
Excellent attention to detail and organizational skills.
Proficiency in electronic medical record documentation.
Strong communication and interpersonal skills.
