Position: Individual Support Specialist - Service Planning

Summary: Block Institute is seeking a dedicated Individual Support Specialist to join our Adult Day Services team. As an Individual Support Specialist, you will collaborate with the Interdisciplinary Team (IDT) in our day program to develop Day Program Staff Action Plans based on CQL Personal Outcome Measures (POMs) and valued outcomes identified by the individual in their Life Plan document. This role involves ensuring that our program adheres to person-centered practices and providing relevant information to Direct Support Professionals to help individuals achieve maximum levels of independence. You'll also be responsible for managing electronic case records through THERAP.

Key Responsibilities:

Ensure accurate Life Plans/ISP and LOC are on file, maintaining and reviewing electronic case records quarterly.
Provide individual program planning, including assessment, development, implementation, and monitoring of creative training programs.
Create and update Day Hab Staff Action Plans, schedule and attend program planning meetings, and collaborate on weekly billing submissions.
Conduct semi-annual Life Planning Team meetings with the residential IDT teams, care managers, CABs, and advocates.
Train Direct Support Professionals (DSPs) on service implementation, ensuring program compliance with OPWDD regulations and Block Institute standards.
Coordinate community inclusion trips for program participants, ensuring at least one outing per month.
Organize morning and afternoon activities with DSPIIIs/Room Leaders.
Review billing ISP data entries and daily attendance records for accuracy.
Develop working relationships with other providers to ensure service provision is person-centered and individualized.
Maintain confidentiality and promote a positive and professional workplace.
Ensure participants are clean and safe at all times, with no inappropriate supervision lapses.

Qualifications:

Bachelor's degree in health, human services, or special education (Master's degree preferred).
Minimum of three years' experience working with individuals with intellectual disabilities.
Supervisory and training experience preferred.
Proficiency in Microsoft Word, Excel, and Therap software.
Working knowledge of NYS OPWDD Day program regulations.
Training in the Council for Quality Leadership’s Personal Outcome Measures evaluation tool preferred.
