*Hours**:* Part-time | 2-3 Days per week | Days

*Some functions of what you will do as a Home Care Occupational Therapist:*
* Completes initial and ongoing comprehensive assessments at appropriate time points as assigned.
* Participates in ongoing interdisciplinary assessment of the patient.
* Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
* Provides services that are ordered by the physician as indicated in the plan of care.

*A few must-haves for this position**:*
* Current licensure in the State of Minnesota as an Occupational Therapist (OT).
* In the last five (5) years, two (2) years of experience as an occupational therapist preferred.
* Able to travel within geographic area serviced by office. Valid drivers license, and proof of auto insurance required.
* CPR Certification
