Responsibilities of the Nurse Practitioner - Health Risk Assessments:

  • Conduct comprehensive in-home health risk assessments on Medicare Advantage members
  • Identify all active and chronic disease conditions to determine physical, mental, and social needs during the visit
  • Educate and counsel patients on identified conditions and screening procedures
  • Document findings using the provided iPad
  • No continuum of care, prescribing, on-call, or follow-up is required

Requirements of the Nurse Practitioner - Health Risk Assessments:

  • Active and unencumbered Nurse Practitioner license
  • Minimum availability of 16 hours per week (including one full 8-hour day) is required to qualify

FAQs:

  • Mileage (if applicable), training, and no-show visits are paid
  • Travel Radius - Flexible; more availability and more of a radius will get a priority
  • Zip Code exclusion - Yes, you can exclude specific zip codes
  • No contact with the patient's PCP - notes are sent electronically
  • A collaborating Physician is assigned, and Malpractice insurance is provided
