Here at Mission Home Health, our PTs provide incredible care to help our home health patients to restore their functional status. Through coordination with care teams, Physical Therapists can greatly increase the function and ability of those in need. Join our team today to see this impact on your community

Duties:

1. Provides physical therapy services to patients according to a written physician's plan of care.

2. Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.

3. Directing physical therapy treatment.

4. Instructing patients and families/caregivers in the use and care of therapeutic appliances.

5. Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.

Pay rate starting at $60 per hour, DOE

Requirements:

Possesses a degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization.
Licensed to practice as physical therapist within the State.
Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
Demonstrates good verbal and written communication and organization skills.
Possess and maintains current BLS Hands On CPR Certification.
Travel – This is a traveling position. Must maintain a valid driver’s license, active auto insurance meeting state minimum requirements and an operational vehicle.
