Job Description:


Provide individualized therapy services to patients in a skilled nursing facility.
Assess patient needs and develop personalized treatment plans to enhance daily living skills.
Collaborate with interdisciplinary teams to improve patient outcomes.
Document patient progress and adjust therapy plans as necessary.
Educate patients and their families on therapeutic exercises and care strategies.

Qualifications:


License to practice occupational therapy in NY.
Experience in a skilled nursing or rehabilitation setting preferred.
Strong communication and interpersonal skills.
