Key Responsibilities:
Offer counseling and support to assist patients and families in coping with the emotional and social aspects of cardiovascular conditions.
Work closely with cardiologists, nurses, and other healthcare providers to coordinate holistic care.
Link patients and families with community resources and support groups.
Educate patients and families on cardiovascular health, treatment plans, and lifestyle adjustments.
Advocate for patient rights and needs within the healthcare system.
Care coordination and follow up are main functions of the role.
_ Home visits are required, throughout San Diego County (will include both North and South County) - this is a major function of the position _
Qualifications:
Current California LCSW license.
Minimum of two years of clinical social work experience, preferably in a healthcare setting dealing with chronic illnesses or cardiology.
Demonstrated ability to handle complex emotional and social situations sensitively and professionally.
Strong organizational and communication skills.
Knowledge of California healthcare regulations and patient rights.
