Brief Overview of Responsibilities for Home Health Clinical Manager:

Supervise field clinicians and all relevant activities pertaining to episodic management and clinical outcomes and provide progressive discipline as required
Receive case referrals and assess patients needs to assign appropriate clinicians
Ensures accuracy with staff to comply with company policy and procedures
Provide staff development opportunities to include in-services and educational classes
Review and investigate the clinical content, diagnoses, medical history, medications, fee status and other procedures from the records of clients.
Appraise staff performance during their probationary period and annually with the Director of Nursing/Administrator, provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.
Identify and evaluate client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff.
Review each nurse's schedule for recertification, discharges, and attendant supervisory visits.
May be requested by Director of Nursing/Administrator to fill in for field staff when necessary.
Manage staff by review weekly reports of productivity, length of stay, hospitalizations, outcomes, and timeliness of care.
Other duties as assigned.

Qualifications:

Preferably two years of home health care experience
At least 1 year of management/leadership expeirence, preferable in home health

Current Virginia State RN License
