The schedule for this role is structured as 7 On/7 Off, with shifts from Monday to Thursday 5 PM to 8 AM, and Friday 5 PM to Monday 8 AM.

Key Responsibilities

The On-Call Registered Nurse is responsible for organizing and directing hospice care, bringing a wealth of experience in nursing with a focus on community health education.

This professional will:

  • Be available to respond to Triage calls during assigned shifts, making visits to patients experiencing changing conditions.
  • Conduct initial, comprehensive, and ongoing assessments of patients and families to identify hospice needs for after-hours calls.
  • Provide thorough physical assessments and document the history of current and previous illnesses.
Nursing Care Delivery

The nurse will utilize all elements of the nursing process to deliver professional care:

  • Assess and evaluate patient status by initiating and revising care plans as necessary.
  • Develop care plans that establish goals based on nursing diagnoses, incorporating palliative nursing actions.
  • Administer medications and treatments as prescribed by the physician.
  • Provide health care instructions to patients and families based on assessments.
Patient Support

Assist patients with daily living activities and promote self-sufficiency and comfort care.

After-hours responsibilities include:

  • Being available for all patient care-related calls and making necessary visits.
  • Providing support to patients and families during crisis periods.
Communication and Coordination

Maintain clear communication with physicians regarding patient needs and changes in conditions. Collaborate with community health professionals to coordinate care plans.

Educate patients and families on self-care techniques and provide guidance on medication, diet, and health counseling.

Interdisciplinary Collaboration

Work closely with the interdisciplinary team to ensure a safe environment for patients and continuity of care.

Participate in team meetings and ensure necessary equipment and services are arranged.

Professional Development

Assume responsibility for personal growth and maintain professional knowledge through continuing education and in-service training.
