About This Position

ChristianaCare is searching for a Health Educator to promote access to services, provide health education and assist community members in navigating the health care and social services systems. K nowledge base in health education and human development is paramount to the success of this role.

NOTE: This position requires reliable transportation as there are multiple locations throughout the New Castle area that this position supports.

Principal duties and responsibilities:

  • Develops and delivers health education for our patients and the community.
  • Creates connections between vulnerable populations, the health care system and community/state resources.
  • Supports patients in the community and/or clinic setting.
  • Utilizes a Social Determinants of Health Screening tool, assists patients to identify socio-economic issues affecting overall health and develop health/social management plans and goals.
  • Facilitates healthcare and social service system navigation.
  • Establishes and maintains professional relationships with community leaders, faith leaders and others responsible for providing services to the community.
  • Participates in community events promoting the office of Health Equity and Cultural Competence.
  • Educates staff, health providers and stakeholders about community needs.
  • Provides culturally appropriate health education on topics related to chronic disease prevention.
  • Advocates for underserved individuals to receive appropriate services. Works to reduce cultural and socio-economic barriers between patients and institutions.
  • Screens and assesses for emotional health conditions impacting overall health and refers to appropriate community resource providers.
  • Collects data and works with leadership in preparing reports.
  • Maintains professional competence by participating in local and national training sessions and conferences.
  • Works collaboratively and effectively within a multidisciplinary team.
  • Establishes positive, supportive relationships with program participants while maintaining transparent communication with collaborating treating caregivers.
  • Builds and sustains positive working relationships with clients, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds.
  • Participates in regular program and department meetings.
  • Performs assigned work safely, adhering to established departmental safety rules and practices, and reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
  • Perform other related duties as assigned and required.

Education and experience requirements:

  • Bachelor's degree preferred.
  • One-year community and patient experience preferred.
  • Knowledge base in health education and human development required
  • Computer experience required.
  • Demonstrated ability to work with diverse populations required.
  • Public health experience is preferred.
  • Bi-lingual language skills are preferred.
  • An equivalent combination of education and experience may be substituted.
