Job Summary
The Family Case Planner will be working in a child welfare prevention program that specializes in medically fragile children and/or adults that have children at risk of being placed. This is a casework counseling and case management position to reduce risk to children.The office is located in midtown Manhattan and is both in office and remote and requires home visits to the assess the safety of the children.
You are a good fit if you have:
Minimum of Bachelor's degree or Masters in social work or MHC or Art Therapy
Child welfare experience working with children and families
Must be willing to do home visits in all 5 boroughs in NYC
Good verbal and written communication skills.
Ability to learn the SBC model to partner with families to establish Action Plans and assist them to reach their goals.
The goal of preventive services is to stabilize and provide support so that children can remain in their homes.
