ChristianaCare HomeHealth is hiring a Full Time LPN Liaison. The Nurse Liaison will identify patients' home care needs, ensuring a smooth transition from acute care and post acute care.

Requirements:

  • LPN licensed to work in Delaware
  • 1 year experience in home health, hospice or care coordination preferred
  • Ability to work and travel to post acute care settings
