As a Hospice Medical Social Worker, here's a big-picture view of what you'll do:

* Provide home-based social services to patients dealing with end-of-life issues including care management, pain, symptoms associated with chronic and terminal illnesses, depression, anxiety, poor nutrition or a lack of resources that make managing their journey difficult
* Work as part of a hospice team which will include an RN, LPN, CNA, Spiritual Counselor, Volunteers, Hospice Aides, PT, OT or SLP, focused on the patient's plan of care and individual goals
* Monitor a patient's psychosocial condition and identify social and emotional needs
* Conduct patient assessments, document progress and ensure patient is moving toward goals
* Provide counseling, community resource planning, crisis intervention and advocacy
* Consult with family and caregivers on patient's plan of care and how to help them progress

A few must-haves for Hospice Medical Social Workers:

* Graduate of a master's program in social work accredited by the Council on Social Work Education
* Licensed to practice as a Social Worker in the state in which employed. The license (LMSW or LLMSW) is active and in
* good standing
* Minimum of one (1) year of experience in health care
* Hospice or homecare experience preferred
* Reliable transportation, including a valid driver's license and car insurance
