The Gardens Health and Rehab Center is looking for Registered Physical Therapists (RPT) to join our Team Shift: PRN ABOUT THE GARDENS HEALTH & REHAB CENTER- Located in Daytona Beach, FL, we are a skilled nursing and rehabilitation facility that is dedicated to helping our residents maximize their potential and live their lives to the fullest. The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff. Current Florida License as a PT (Physical Therapist) At least one (1) experience working in a Skilled Nursing Facility preferred. Provides timely written documentation per facility and department requirements and as required by insurance provider to meet licensing, certification, regulatory, and reimbursement requirements. Supports and participates in department operations and development. Reports any changes in patient status or needs to Director of Rehab and interdisciplinary team as appropriate. Supervises Physical Therapy Assistants with the goal of ensuring optimal patient care and achievement of outcomes. %{{
