Adoration Home Health Care is seeking an experienced, compassionate, and committed LPN to join our team in Reidsville, NC This role offers a flexible schedule and the chance to expand your nursing skills while delivering crucial one-on-one care to patients in their homes and supporting their families. You will make a difference in our patient's lives by providing safe patient care, deliver individualized nursing care plans to ensure our patients have an optimal care experience while working as part of an interdisciplinary team, supporting both patients and their families. Location: Reidsville Branch (Wentworth)

Coverage area: Rockingham County + 50 mile radius of Branch

Schedule: PRN

Must have: Valid drivers license, reliable vehicle, and proof of car insurance

Responsibilities:
Communicates health/nursing needs and/or recommendations to RN supervisor
Coordinates admissions, discharges, and follow-up as needed
Assists in medical/health appointments as needed
Ensures the provision of nursing/health services under the direction of the RN and as ordered by the physician
Provides direct care as outlined in the Service Plan/Nursing Care Plan
Takes and records vital signs, weights, wound measurements, etc.
Identifies trends outside of parameters, or in need of attention and communicates to RN/physician
Qualifications:
Must be a Licensed Practical/Vocational Nurse (LPN or LVN, per state licensure requirements) in good standing
CPR certification
Six (6) months of direct nursing experience
