*Job Summary*

We are seeking a dedicated Licensed Practical Nurse to join our healthcare team. The ideal candidate will provide compassionate care to patients and support our medical staff in delivering high-quality healthcare services.

*Responsibilities*
* Perform nursing procedures under the supervision of a registered nurse or physician
* Administer medications and treatments as prescribed by the healthcare provider
* Monitor patient vital signs and report any changes to the healthcare team
* Assist in patient assessments and care planning
* Maintain accurate medical records and documentation
* Educate patients and their families on healthcare procedures and treatments
* Collaborate with other healthcare professionals to ensure comprehensive patient care

*Required: *

1-3 years' experience required

Will take someone with SNF experience, not just LTC

EMR: Point Click

Common patient diagnosis: Dementia, Chronic Diseases, Diabetes

*Hours*

Will take someone for 11a-11p if they are interested

8-hour evenings

Shift: 3p-1130p

Pay: $40.00 - $46.00 per hour
