Description

The Care Manager RN is responsible for managing patient populations with chronic illness and/or multiple comorbidities. This individual works in coordination with multiple PCPs and serves as the central link to an identified population of patients. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the RN Care Navigator assists patients to gain self efficacy/management skills, achieve optimum functional health status, and quality of life.

Required Qualifications:

  • Graduation from an accredited school of nursing.
  • upon hire: Washington Registered Nurse License.
  • 2 years Work experience in RN role.

Preferred Qualifications:

  • Bachelor's Degree Nursing.
  • Experience managing complex patients in an ambulatory setting.
  • Medical Home Model experience.

About the Team

Providence Clinical Network (PCN) is a service line within Providence serving patients across seven states with quality, compassionate, coordinated care. Collectively, our medical groups and affiliate practices are the third largest group in the country with over 11,000 providers, 900 clinics and 30,000 caregivers.
