As a member of a multidisciplinary health care team, the ambulatory clinical pharmacist provides distributive, clinical, consultative, and/or administrative pharmaceutical services. The clinical pharmacist selects, dispenses, drugs, medicines, and other therapeutic agents.
Essential FunctionsThe essential functions listed are typical examples of work performed by positions in this job classification. Must perform all duties and responsibilities in accordance with the hospital’s policies and procedures, including its Service Standards and its Code of Conduct.
Identifies fine differences between drugs as they relate to therapeutic efficacy of the products.
Applies knowledge of the physical and chemical properties and characteristics of computing dosages, weighing and measuring drugs, and mixing and compounding drugs and chemicals.
Applies knowledge of solubility behavior of substances, stability of drugs, usages of drugs, and limitations on modes of administering drugs, and similarly applies knowledge of incompatibilities in evaluating dosages and permissible concentrations of drugs, and in anticipating, preventing, and treating undesirable chemical reactions.
Has legal responsibility for proper interpretation of prescriptions and the dispensing of prescribed products, and call to physician's attention any synergistic, antagonistic, and cumulative effects, as well as questionable overdoses or incompatibilities.
Specifies proper storage, protection, of therapeutic agents, in accordance with the standards of the United States Pharmacopoeia (U.S.P.) and the manufacturer package insert.
Keeps continually abreast of new developments in the fast-moving drug field and be aware of emerging patterns, systems, and techniques in pharmacy practice.
Participates in the training or preceptorship and education of pharmacy residents, interns, and pharmacy students.
Maintains records, documents patient data, prepares written reports, and provides oral reports concerning patient care.
Requests prior authorization with all requested supporting documentation.
Prepares claims for billing including obtaining all supportive documentation such as prescriptions, progress notes, lab reports, prior authorizations and other information which may be required by third party payers to reimburse for services.
Investigates patient assistant programs and assists patients with enrollment.
Provides patient/caregiver medication counseling.
Provides clinical supervision of pharmacy technicians.
Provides non-sterile hazardous and non-hazardous product preparation oversight.
Management and control of controlled substances.
Assesses and escalates medication concerns to providers as needed.
Contacts patients for specialty medication refill calls and coordinates non-specialty medication refills.
Packages medication per storage requirement and arrange for deliveries.
Writes standard operating procedures, master formulas or staff communications.
Participates in committees and projects as assigned.
May participate in hospital research projects or perform pharmaceutical research.
May perform the following higher-level functions, under the supervision of a higher-level pharmacist:
Performs patient clinical assessment calls and specialty medication refill calls.
Coordinates complicated discharges and performs discharge teaching.
Negotiates payer letters of agreement for provision of pharmacy services.
Requests complicated prior authorizations and if denied, work through appeal process with providers.
Oversees packaging of medication per storage requirements for delivery.
Minimum QualificationsAny combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.
Education: Doctor of Pharmacy degree from a School of Pharmacy accredited by the American Council on Pharmaceutical Education.
Experience: Two (2) years related experience.
Licensure/ Certification: Licensed as a Pharmacist by the California Board of Pharmacy.

Current American Heart Association (AHA) Certification for Basic Life Support (BLS) for healthcare providers.
Knowledge, Skills, & AbilitiesThese are the observable and measurable attributes and skills required to perform successfully the essential functions of the job and are generally demonstrated through qualifying experience, education, or licensure/certification.
Knowledge of prescription and non-prescription drugs and drug products and their indications, efficacy, dosage, mechanisms of action; fate and disposition (if known), major contraindications and potential side effects.
Knowledge of Medical terminology and abbreviations; pharmacology and pharmacological nomenclature; medical therapeutics, pharmacokinetics, and disease states.
Knowledge of methods and procedures involved in proper handling and storing of pharmaceutical supplies.
Knowledge of influence of patient's age, sex, concomitant disease states, concurrent drug therapy, foods and diagnostic procedures on drug activity and disposition.
Knowledge of physiology, anatomy, pathology, chemistry and mathematics as they relate to pharmacy practice.
Knowledge of local, state and federal regulatory requirement related to the functional area.
Ability to apply judgment and make informed decisions.
Ability to plan, organize, prioritize, work independently and meet deadlines.
Ability to foster effective working relationships and build consensus with other departments and external vendors.
Ability to communicate effectively with others and exercise a high degree of tact in coordinating with and advising other members of hospital staff, patients, and various other professional contacts.
Maintain familiarity and operational knowledge of the departmental information and technical systems.

Pay Range
Compensation is based on the level and requirements of the role. Salary within our ranges may also be determined by your education, experience, knowledge, skills, location, and abilities, as required by the role, as well as internal equity and alignment with market data.
Typically, new team members join at the minimum to mid salary range.
Minimum to Midpoint Range (1.0 FTE): $187,158.40 to $203,070.40.
