Fort Behavioral Person-centered Therapy is seeking a dedicated Licensed Clinical Social Worker (LCSW) for a full-time remote position. In this role, you will be instrumental in enhancing the psychosocial well-being of clients, particularly seniors, by providing tailored support and interventions. This position offers a competitive salary range of USD 70,000 to 100,000 per year, depending on experience and qualifications.

Work Environment

This full-time remote position allows you to work from the comfort of your home while remaining part of a collaborative and dynamic team committed to patient-centered care. The work setting predominantly involves the Hospice Center, where you will engage with seniors at critical stages of their lives. While the standard hours are Monday through Friday from 8:00 AM to 4:30 PM, flexibility is key, as you may need to accommodate occasional evenings or weekends based on client needs.

Key Responsibilities
  • Conduct comprehensive assessments to identify the psychosocial needs of senior clients, integrating their emotional, mental, and social factors into a coherent evaluation.
  • Collaborate closely with clients and their families to develop personalized care plans that align with the individual’s specific goals, preferences, and health challenges.
  • Provide counseling and emotional support, helping clients navigate significant life transitions, including grief, loss, and adaptations in health conditions.
  • Advocate for clients, ensuring their rights are upheld and that they have access to necessary community resources and support services, facilitating their journey toward stability and wellness.
  • Work in conjunction with interdisciplinary teams—composed of nurses, caregivers, and healthcare providers—to enhance care coordination and ensure holistic treatment approaches.
  • Perform regular follow-up visits, offering continuous monitoring of clients’ well-being, assessing their living conditions, and promptly addressing any identified concerns or safety issues.
  • Facilitate open communication and conflict resolution among family members involved in client care, promoting a healthy support system.
  • Educate clients and families regarding available community resources, support groups, and enrichment programs that enhance social engagement and improve quality of life.
  • Maintain meticulous records of assessments, care plans, progress notes, and all relevant interactions to ensure continuity of care and compliance with regulatory standards.
  • Stay updated on best practices within the field of social work, ensuring adherence to ethical standards and current social work policies.
Patient Population / Client Focus

This position primarily focuses on the senior population, including those receiving hospice care. We recognize the multifaceted needs of elderly clients, especially concerning mental health, social interactions, and emotional support during life transitions. Qualifications
  • A Master’s degree in Social Work from an accredited institution is required for this position.
  • Current and valid state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • A minimum of two years of relevant experience in a social work setting, ideally within community services or a geriatric-focused role.
  • Comprehensive understanding of social work principles, issues pertinent to aging populations, and resources available to support seniors and their families.
