Reporting to the Regional Director of Case Management, the Assistant Director of Case Management will will lead and manage the Case Management department, including supervising staff and ensuring compliance with all regulations and policies. They will develop and implement strategies to enhance patient care coordination and resource utilization, collaborating with medical staff, patients, and families to create personalized care plans and ensure seamless transitions of care. This role involves overseeing discharge planning, home healthcare arrangements, and access to community resources, as well as monitoring and analyzing department performance to make data-driven decisions that improve processes and outcomes. The Director will also provide leadership in managing complex cases, ensuring effective communication and coordination among all stakeholders, and staying informed about industry trends and best practices to integrate them into department operations.
The ideal candidate for this position will have a Master's in Nursing or Social work and multiple years of leadership experience in Case Management in a hospital setting.
