Responsibilities:

  • Provide primary care services in post-acute care facilities, including skilled nursing and assisted living.
  • Drive high-quality, safe, and cost-effective patient care.
  • Collaborate with interdisciplinary teams and follow CMS guidelines.
  • Work closely with hospice, palliative care, and home care agencies for safe patient transitions.
  • Provide oversight, guidance, and mentorship to clinicians within facilities.

Opportunity Overview:

  • Schedule: Monday through Friday.
  • Patient Volume: 15-20 patients per day.
  • Compensation: our compensation is a productivity-based model (fee for service) that offers you control to maximize your income potential; expected compensation is estimated salary range of $171,000 to $277,000 annually; this full-time role is benefit eligible.

Qualifications:

  • Preferred physicians with a background in internal medicine (IM) or family medicine (FM).
  • Physicians with other backgrounds who have a passion to get into post-acute care are welcome to apply.
  • New Graduates welcome to apply.
