Charter Health PC is seeking a Licensed Clinical Social Worker to join our team. We are an internal medicine and behavioral health practice. Our services include primary care, behavioral heatlh, hospital medicine and nursing home medicine. We are a private practice member of Mass General Brigham Community Physicians.

We are seeking to add an LICSW to our behavioral health team. This individual would work closely with a psychiatric nurse practitioner to provide 1:1 psychotherapy to selected residents at skilled nursing facilities. Additional visits may be available within our internal medicine office located in Salem, MA.

The position is 20-40 hours a week depending on what the candidate is seeking and we can offer some flexibility in terms of hours/days.

The ideal candidate will have a strong background in psychotherapy and work with behavioral health and especially with adults/older adults. The position requires the ability to travel between practice sites (all within the Salem/Peabody/Danvers area). Experience with billing for psychotherapy is a plus.

\*Responsibilities\*

Conduct assessments and develop treatment plans for clients - Provide individual and group therapy sessions - Collaborate with physicians, nurse practitioners and other healthcare professionals to ensure comprehensive care - Advocate for clients and connect them with appropriate resources - Maintain accurate and up-to-date client records.

\*Experience\*

Must have experience in psychotherapy - Medicare regulations and procedures - Behavioral health - Strong background in social work.

\*Qualifications\*

· Master's degree from an accredited college or university

· State professional LICSW license

· Valid driver’s license and ability to travel between locations

· Ability to work independently on a daily basis

· Excellent written, verbal, interpersonal and organizational skills

· Ability to use email, the internet, and to learn other healthcare related software (Epic, PCC, MatrixCare).

Job Types: Full-time, Part-time

Pay: $45.00 - $65.00 per hour

Expected hours: 20 – 40 per week
