Job Description

Pride Health is hiring a LPN to support our client’s medical facility in New York and other locations. This is a 26-week assignment (Possibility of extension or conversion) opportunity and a great way to start working with a top-tier healthcare organization

Job Summary:

A Licensed Practical Nurse (LPN) working in a Skilled Nursing Facility (SNF) provides essential patient care to residents who require long-term, rehabilitative, or post-acute care. The LPN is responsible for administering medications, monitoring vital signs, assisting with daily activities, and documenting patient conditions in compliance with facility policies. They collaborate closely with Registered Nurses (RNs), physicians, and other healthcare professionals to develop and implement individualized care plans. The role requires the LPN to provide compassionate, patient-centered care while ensuring the safety and well-being of all residents. Additionally, LPNs in SNFs may assist in staff training and contribute to maintaining a clean and supportive environment for patients. Strong communication and a thorough understanding of clinical procedures are vital to success in this position.


Key Responsibilities:

  • Administer medications and treatments as prescribed.
  • Monitor and record vital signs, reporting changes to the team.
  • Provide basic nursing care, including wound and catheter care.
  • Assist with activities of daily living (ADLs) like bathing and dressing.
  • Document patient conditions and treatments accurately.
  • Ensure patient safety and follow infection control procedures.
  • Communicate with patients, families, and the care team.
  • Supervise and mentor nursing assistants.
  • Respond to emergency situations as needed. Participate in ongoing training and education.

Qualifications:

  • BLS/ACLS certifications required.
  • Active LPN License of New York state
  • Strong critical thinking, communication, and stress management skills.

  • Job Type: 26 Weeks Contract
  • Shifts: 8 HR SHIFT (DAY/EVENING)
  • Weekly Pay: $1270 to $1470

*Offered pay rate will be based on education, experience, and healthcare credentials.
