Job Title: LTC Certified Nurse Assistant
Department: LTC Nursing Service
PART I: POSITION OVERVIEW
Position Summary:
The certified nursing assistant in long term care assists in the provision of nursing care of the residents as directed by the CHARGE NURSE/LPN charge nurse. Care skills include but are not limited to: comfort and safety measures, personal hygiene, assisting with activities of daily living, and simple nursing procedures. When students are providing resident care, treatment, and or services as part of their training. Any or all employees may be asked to work and supervise the students.
Employees under the age of 18 will be prohibited from operating patient lifts or hoists. The employee who is under age 18 may participate in patient/resident positioning and stabilization during application/removal of the sling, harness or belt to the lift by another staff member. He/She may also propel the lift from one location to another so long as the employee under the age of 18 is not the person operating the lift controls.
Education/Experience:
High school diploma or equivalent desired.
Required Credentials (Licensure, Certification, or Registration):
Certification from a state approved program for nursing assistant, or completion of in house certification program by the end of four months of employment.
Employment Variables:
Scheduled work hours may vary. Required to rotate weekends and holidays. Occasionally may need to work beyond normal working hours. Initial Tuberculosis (TB) test and drug screening is required by Winner Regional Health. Rubella titer will also be drawn upon hire and immunization is required if no past exposure or indication of immunization.
Required to wear name tag provided by WRH and to follow the dress code of WRH.

PART III: ESSENTIAL FUNCTIONS
Essential functions are critical or fundamental to the performance of the job. They are the major functions for which the person in the job is held accountable. Following are the essential functions of the job, along with the corresponding performance standards.
Duties and Responsibilities
Provides timely and proper notification for absences and paid personal leave. Finds a replacement when necessary. No unexcused absences or excessive tardiness. Knows emergency procedures for fire, internal/external disaster plans, codes, etc. and participates in drills. Verbally supportive of Winner Regional Health and the Sanford Health Network. Participates on committees and/or is involved in special projects of Winner Regional Health. Provides for patient, resident, employee, and visitor safety through preventative measures. Identifies hazards and reports incidents. Utilizes and adheres to appropriate precautions/techniques with hazardous chemicals, blood and body fluids, medical waste and/or equipment. Satisfactory attendance at department meetings (50% attendance meets expectations). Is cross-trained to other areas/positions/departments to provide assistance when needed.
Clinical Competency
Demonstrates operating knowledge of equipment and supplies necessary to perform assigned duties. Demonstrates appropriate aseptic and hygiene techniques to prevent the spread of infection to resident, self and others. Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care. Assists Charge nurses and Physicians with assessments, examinations and treatments.
Resident Care Functions
Assists in development and implementation of resident plan of care including personal hygiene, nutrition, and elimination. Observes residents for reactions to treatments or any unusual conditions and records or reports adverse conditions to the Charge nurse.
Ancillary Activities
Orders late trays and snacks from dietary. Assists in cost containment through the appropriate ordering and conserving of supplies and equipment. Monitors supplies and equipment and orders through WRH as needed.
Resident & Work Safety
Demonstrates appropriate lifting, repositioning, proper use of restraints, and moving of residents and equipment. Maintains good hand washing practices before and after resident care. Reports resident concerns, c/o pain and abnormal vital signs to the charge nurse. Promotes a safe and hazard free resident care environment.
Working With Delegation
Willingly accepts from charge nurses the delegation of tasks that are within the job description and own competence level. Consistently seeks clarity and/or direction from charge nurse when tasks or assignments are unclear, outside job description, or outside the competency level. Seeks appropriate assistance when needed from healthcare team members to ensure the safe administration of resident. Consistently provides charge nurse with feedback regarding outcomes from delegated tasks (i.e., VS, pain assessment, and activity). Demonstrates appropriate independence/interdependence in the provision and documentation of resident care.
PART IV: COMPLIANCE
Compliance
