As an Interim RN Case Manager, you will play a crucial role in coordinating patient care and ensuring the best possible outcomes for patients. You will collaborate with a multidisciplinary team to develop and implement care plans, facilitate communication between patients and healthcare providers, and ensure that patients receive appropriate, timely, and cost-effective care. Key Responsibilities:

  • Conduct comprehensive patient assessments to identify needs and develop individualized care plans.
  • Coordinate and monitor patient care from admission through discharge and follow-up.
  • Collaborate with healthcare providers, patients, and families to ensure effective communication and care coordination.
  • Facilitate access to necessary services and resources, including community services and home care.
  • Ensure compliance with all regulatory requirements and hospital policies.
  • Advocate for patients’ rights and needs, promoting patient-centered care and optimal outcomes.

Requirements:

  • Current Registered Nurse (RN) license in New York State.
  • Bachelor’s degree in Nursing (BSN) preferred.
  • Minimum of three (3) years of acute care hospital experience.
  • At least two (2) years of experience in case management.
  • Strong critical thinking and problem-solving skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
