Registered Nurse Case Manager

The RN/Occupational Medicine Case Manager role is to be a central resource for injured workers, employers, healthcare providers, claims adjustors and St. Luke's Preemployment physical applicants to assist in the communication and coordination of the care for the injured worker and facilitating results for the physical applicant. The case manager will provide support for quality cost-effective care that is reasonable and necessary and related to the injury with the goal of assisting the injured worker with return to gainful employment. The case manager will also assist in the processing and communicating of physical exam results to St. Luke's Human Resources.
JOB DUTIES AND RESPONSIBILITIES:
Provide communication from the initial notice of injury to case closure
Coordination of care by scheduling diagnostic studies, PT/OT and specialty referrals
Meeting and touring employer sites
Care Now site visits and calls to meet or discuss cases/physicals with providers and staff
Tracking all St. Luke's Preemployment Physicals
Monitoring and assist in processing of Work queue referrals to Occupational Medicine
Sending medical records to appropriate contacts for work injuries and physicals.
Making outreach calls to St. Luke's applicants who are given a MRF to have the treating provider complete
Communicating updates to Human Resources and or Talent Acquisition
Other duties may be assigned
PHYSICAL AND SENSORY REQUIREMENTS:
Constant sitting or up to 8 hours/day. Occasional walking on even and uneven surfaces. Occasional standing. Occasional stair climbing. Occasional bending, squatting, kneeling, and work above shoulder level. Lifting, Pushing, Pulling and Carrying up to 25 pounds. Occasional driving. Ability to see and hear normal conversation. Use of phone and computer. Writing.
EDUCATION:
Registered Nurse with current license to practice in the State of Pennsylvania or seeking PA licensure through reciprocity. Current license to practice in the State of New Jersey or seeking NJ licensure through reciprocity.
TRAINING AND EXPERIENCE:
Two years of clinical nursing experience required. Case Management or Occupational Medicine experience is beneficial.
