Job Overview:
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for coordinating and managing the care of patients, ensuring they receive the highest quality of medical services. Duties:
- Conduct patient assessments to determine their healthcare needs and develop individualized care plans
- Collaborate with healthcare professionals to coordinate and manage patient care
- Monitor patient progress and make necessary adjustments to care plans
- Educate patients and their families on medical conditions, treatment options, and self-care techniques
- Administer medications and treatments as prescribed by physicians
- Perform diagnostic evaluations and interpret test results
- Monitor vital signs and assess overall patient well-being
- Provide emotional support to patients and their families
- Document all patient interactions, treatments, and progress in medical records
- Ensure compliance with healthcare regulations and protocols

Requirements:
- Valid Registered Nurse (RN) license
- Experience with both home health or hospice preferred
- In-depth knowledge of medical terminology, physiology, and diagnostic evaluation
- Strong critical thinking and problem-solving skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Familiarity with managed care principles and practices
- Compassionate and empathetic attitude towards patients
- Strong organizational skills with attention to detail

Job Type: Full-time

Pay: $35.00 - $45.00 per hour

Expected hours: 5 - 25 per week
