Job Description - Staff Nurse RN (0.8 FTE) - Senior Care / Extended Care Clinics (242124)

Hennepin Healthcare is hiring a Staff Nurse RN (0.8 FTE: 4 days/week, 8 hour shifts) for our Senior/Extended Care Clinics in Minneapolis to work day shift (Mon-Fri). A sign-on bonus is available for eligible external candidates (current employees and former employees returning to HHS within one year are ineligible).

Responsibilities

  • Protecting, promoting wellness and optimizing health
  • Triaging patients by assessing severity of symptoms and referral to appropriate setting or level of care
  • Alleviating suffering through the treatment of the human response
  • Partnering with the patient, their family and significant others
  • Documenting interventions and an individualized plan of care
  • Educating patient on their disease process
  • Assuring continuity of care
  • Educating patient and family in preventative care which promotes health and well being

Qualifications

Minimum Qualifications:

  • Graduation from an accredited school of nursing
  • Possession of a valid license as a Registered Nurse issued by the State of Minnesota

Preferred Qualifications:

  • Certification in area of clinical specialty
  • Two years of ambulatory experience
  • Prior experience working in Long Term care
  • BSN Preferred. Bachelor of Nursing degree is strongly encouraged. Advancement in academic education will be discussed as part of individual professional development during the annual performance review.
