RN Care Management Coordinator

Pay Range
Days: $30.60 - $35.47
The base pay range is a good faith representation of current potential base pay for successful applicants. Shift differentials have been included in the range. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity and contracts
Minimum Education Requirement :
Graduate of an accredited school of nursing.
Minimum Experience Requirement :
2 years' experience in clinical or community resource settings required. Care coordination and/or case management experience is desirable.
Evidence of essential leadership, communication, education, and counseling skills. Proficiency in communication technologies (email, cell phone, etc.) Highly organized with ability to keep accurate notes and records. Experience with health IT systems and reports is desirable
Local knowledge about and connections to community health care and social welfare resources is desirable
Must possess and demonstrate knowledge of the growth and development of varied age groups, neonatal to geriatric. Demonstrated ability to handle confidential hospital information. Well developed critical thinking skills. Knowledge and understanding of medical terminology. Ability to communicate effectively with medical professionals of all disciplines. Clear verbal and written communication skills, excellent phone etiquette. Ability to identify potential and actual patient occurrences that may be reportable incidents or those requiring further internal investigation.
Minimum License Requirement :
Current NYS licensure as an RN. BLS certified
