Apply now and discover why you were made for this Our Hospice Registered Nurse Case Managers (RNs) enjoy some notable benefits: Salary: $55.00 - $60.00 per hour with a sign-on bonus up to $15,000Available position: Full Time - Monday - Friday - day shift, with occasional on call1:1 nurse-to-patient ratios where you have an impactAutonomy and work-life balanceOnline training, growth and ability to earn CEUsTuition discounts through Rasmussen UniversityGenerous PTO, Holiday Pay, Medical/Dental/Vision and 401(k) Benefits As a Hospice Registered Nurse Case Managers (RN), here’s a big-picture view of what you’ll do: Conduct comprehensive assessments of patients’ physical, emotional, psychosocial, and spiritual needs to develop individualized care plans.Coordinate interdisciplinary care team meetings to ensure collaboration among healthcare professionals involved in the patient’s care, including physicians, social workers, chaplains, and volunteers.Educate patients and their families about disease progression, symptom management, medication management, and end-of-life care options to empower them to make informed decisions.Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and outcomes in compliance with regulatory requirements and organizational policies.Provide compassionate end-of-life care, including symptom management, comfort measures, and support for patients and their families during the dying process.Participate in quality improvement initiatives aimed at enhancing the quality of care provided to hospice patients and their families.
